Communication between clinic visits

For caregivers, completing a child’s immunization visits involves returning to a clinic over time. For health workers, it involves knowing who is due back and which families need follow-up. On this project, that was the connection between visits.

The project combined caregiver reminders with health worker visibility into follow-up. SMS and voice delivery allowed communication through basic phones, with preferred local languages helping make reminders more accessible.

A D-Prize-supported pilot in the Ashanti Region

D-Prize’s public profile describes Synchora Health’s pilot at Juaben Government Hospital, Achiase Health Center, and Bomfa Health Center. Its model combines caregiver reminders with point-of-care tracking in static and mobile child welfare clinics.

The profile sets a goal of reaching at least 300 children over three months. That is a published pilot target, not a claim of measured completion or improved vaccination rates.

How the workflow connects caregivers and health workers

  • Enrollment: frontline teams connect children and caregivers with the reminder workflow.
  • Reminders: SMS, WhatsApp, and voice communication support caregivers in returning for scheduled visits. A caregiver can confirm or decline.
  • Visit tracking: health workers record care at the point of delivery and identify follow-up needs.
  • Continuity: the clinic uses attendance information to organize ongoing outreach.
